By booking an appointment or signing the form in-person, you confirm that you have read, understand, and agree to the following:

1. Assumption of Risk
You acknowledge that receiving permanent jewelry involves welding a chain directly onto your body and may involve inherent risks, including but not limited to injury, burns, allergic reactions, or medical complications. You are voluntarily choosing to proceed with the procedure at your own risk.

You also acknowledge the ongoing risks related to exposure to COVID-19 or other infectious diseases, and that participating in this service may result in illness or other health complications.

2. Health & Safety
By proceeding, you confirm that:

• You are in good health and have no known medical conditions that would prevent you from safely receiving permanent jewelry.
  
• You are not currently experiencing symptoms of COVID-19 or any contagious illness, nor have you knowingly been in close contact with someone who has tested positive in the last 14 days.
  
• You will follow all staff instructions and safety protocols during your appointment.

3. Liability Waiver
You agree to release and hold harmless Sassafras Clothing Co., LLC and its team from any liability for injuries, medical issues, property damage, or financial loss resulting from or related to the permanent jewelry service, including any risk related to illness or emergency medical treatment. This waiver does not apply to claims resulting from gross negligence or willful misconduct as prohibited by Missouri law.

4. Minors
We require parental consent and in-person accompaniment for anyone under 14 years old. The accompanying adult is responsible for ensuring the minor understands the risks and care of their jewelry. Once applied, the jewelry becomes the minor’s responsibility.

5. Reweld Policy
Free rewelds are offered within 30 days of your appointment if the break occurs at the weld point.

After 30 days, rewelds are $20. Rewelds for chains not purchased from us are $35.

We do not offer replacements, refunds, or discounts for lost jewelry, regardless of the reason. Lifestyle, job, and daily activities can all impact the wear and longevity of your piece.

6. Medical Treatment
If you require medical assistance during your appointment, you authorize Sassafras Clothing Co., LLC to seek such treatment. You are responsible for any associated costs.

7. Governing Law
These terms are governed by the laws of the State of Missouri. Any legal matters arising will be handled in Missouri courts.

8. Right to Refuse Service
Sassafras Clothing Co., LLC reserves the right to withhold permanent jewelry services for any reason. Services will not be provided to any individual who does not accept and agree to these Terms & Conditions.
